American Society of Consultant Pharmacists and American Society for Healthcare Risk Management Join Medication Errors Council

August 1, 1997 — At a recent meeting, the National Coordinating Council for Medication Error Reporting and Prevention (NCC MERP) unanimously accepted the American Society of Consultant Pharmacists (ASCP) and the American Society for Healthcare Risk Management (ASHRM) as regular members.

Each of these national organizations will occupy one full voting seat on the Council for a two-year period. ASCP joins the Council in the category of "Pharmacy Related Organizations" and represents long-term care. ASHRM joins the Council in the category of "Risk Management/Quality Assurance" and represents health care risk management.

In announcing the new appointments, Council Chairperson William M. Ellis commented, "Both ASCP and ASHRM have proven track records for addressing medication errors. Their expertise and involvement with the Council will assist us in carrying out our mission."

As Council members, ASCP and ASHRM will work with the 14 founding member organizations representing medicine, pharmacy, nursing, government, and industry to promote the reporting, understanding, and prevention of medication errors. The Council's specific goals are to:

  • stimulate the development and use of medication error reporting and evaluation systems in individual health care organizations;
  • stimulate reporting to a national system for review, analysis, and development of recommendations to reduce and prevent medication errors;
  • examine and evaluate the causes of medication errors;
  • increase awareness of medication errors and methods of prevention throughout the health care system, which includes health care organizations/facilities, delivery systems, practitioners, manufacturers, regulators, and consumers; and
  • recommend strategies relative to system modifications, practice standards and guidelines, and changes in packaging, labeling, and product identity.
