National Coordinating Council Announces Medication Error Actions

Systems Approach, Labeling/Packaging Recommendations Adopted


July 17, 1998 — The National Coordinating Council for Medication Error Reporting and Prevention has approved two sets of recommendations to help reduce the risk of medication errors due to labeling and packaging of drug products: one for health care organizations and one for health care professionals, announced William M. Ellis, R.Ph., M.S., chair of The Council and Executive Director of the APhA Foundation of the American Pharmaceutical Association.

"The recommendations are part of a comprehensive plan to help reduce medication errors caused by labeling and packaging problems," said Ellis. "The Council's goal is to provide guidance on medication error prevention to all parties in the health care continuum; previously, recommendations for the industry and standard-setters/regulators were issued by the Council. The latest recommendations encourage reporting of actual and potential medication errors to internal, local, and national reporting programs by health care organizations and individual health professionals."
